Information you choose to provide
The consultation form asks for your name, work email, organization, project type, and an optional project overview. The current static form prepares an email addressed to tech@mindaptix.com in your device’s email application. The website does not silently submit that form in the background.
Do not send protected health information
Do not include patient names, medical records, insurance identifiers, treatment details, or other protected health information in a website inquiry. Initial discussions should use non-sensitive business and product information only.
How inquiry information may be used
Information you send may be used to respond to your inquiry, understand the requested service, arrange a consultation, prepare a proposal, maintain business records, prevent misuse, or comply with legal obligations. It should not be sold as a consumer marketing list.
Email and hosting providers
Your email provider and the recipient’s email provider process messages according to their own terms. Once the site is publicly hosted, the hosting provider may process standard technical logs such as IP address, requested URL, browser type, and timestamps for security and service operation. This notice should be reviewed when the final hosting, analytics, CRM, or form-processing services are selected.
Retention and deletion requests
Business inquiries may be retained while they are active and as reasonably needed for records, security, or legal obligations. To request access, correction, or deletion of information you submitted, contact tech@mindaptix.com. Requests may require identity verification.
Updates
This notice may be updated when website functionality or service providers change. The effective date above should be revised when a material update is published.
